COSA registration form (pdf - 65K) If you are a COSA member attending the AEOE conference, please use the AEOE conference registration form, check the COSA box, and add the $20 to your AEOE registration fees. If you are not attending the AEOE conference, use this COSA Only form to register for the COSA Conference. If you are attending COSA only or COSA plus AEOE and plan to arrive on Thursday night, please register for Thursday night on this COSA form.
